Incident Reporting

In the event of an injury at a sanctioned event, an incident report should be filled out by a tournament or club director, a coach, or a USAV Official and submitted to the OKRVA Region office immediately following the sanctioned event.

USA Volleyball accepts claim forms for individuals involved in a reported incident.
