To assign players and adults to a team’s roster, submit the following form using the linked template.

The template contains two tabs.  Complete all fields in each tab and upload the completed spreadsheet (both tabs) through the form below.

• Tab 1 – players, including team, first name, last name, USAV #, and jersey #
• Tab 2 – adults, including first name, last name, USAV #, and role on the team
